I hereby affirm that I read and understand the terms and conditions of this membership agreement without reservation.
I agree to all of the terms and conditions of membership of Akoma Group Church (referred to as the “Church”). I understand that this membership is being issued for a limited period of time and shall expire when such period of time has elapsed. The limited period of time during which my membership shall be valid begins on the date my membership application is approved and ends one year from that date, after which time my membership shall expire. I understand that this Agreement is required to join the Church, and I promise to abide by its terms and conditions at all times during my tenure as a member of the Church. In consideration of the processing of my application, and of the rights, privileges and benefits of membership in the Church, and for other good and valuable consideration the receipt of which is hereby acknowledged, I enter into this Agreement as of the date set forth next to my signature.
By choosing to microdose at Akoma Entheogenic Church, I acknowledge that I am voluntarily participating in this practice and understand the potential risks involved. I agree to take full responsibility for my actions and experiences while microdosing. I hereby release and hold harmless Akoma Entheogenic Church, its staff, and affiliates from any liability, claims, or demands arising from my participation in microdosing activities. I confirm that I am of legal age, in good health, and have no medical conditions that would contraindicate my participation. I understand that this waiver is binding and applies to all future microdosing activities at Akoma Entheogenic Church.
